    FAMILIES - PLEASE BRING ALL PRINTED DOCUMENTS REQUIRED WITH YOU TO THE WELCOME CENTER.

    What is an affidavit? 

    A Residency Affidavit is a legal document that you can use to verify your residence and the residency of anyone living with you. An Affidavit will often be used in response to a school, financial institution, court, or other entity's request for proof of residency.

    Affidavits are provided at the Welcome Center from 8:00 am to 4:00 pm.  The Welcome Center is located at 166 Holly Smith Drive, McDonough, GA 30253. Computer kiosks are also available for information updates, and bilingual staff members are available to families who may need assistance in other languages.     Enrolling Parent/Guardian

    Property Owner or Leaseholder

    Valid Government Issued Photo Identification (can be passport or other photo ID that is not expired)

    Valid Government Issued Photo Identification (can be passport or other photo ID that is not expired)

     

    Printed copy of current Lease/Rental Agreement or Property Tax Bill or Closing Disclosure (Lease/rental agreements must be current and include signature page)

    Printed copy of current Utility Bill or Letter of Service (gas, electric, water, sanitation, cable)

    What are the hours of operation? 

    8:00 am - 4:00 pm (No appointments are necessary)  Extended hours prior to the first day of school.

     

    When are we closed? 

    Our calendar is consistent with the District's calendar.  Summer hours vary.
