• HENRY COUNTY SCHOOL ADMISSION REQUIREMENTS

    ADMISSIONS CHECKLIST

     Proof of Residency (MUST HAVE TO ENROLL):           

    In order to enroll, the enrolling parent must have the following documents in their name:

    1-      Valid Lease or Rental agreement with parent as lease holder and signed by both parties AND
             a.      GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.
     
    OR
     
     2-      Property Tax Bill or Settlement Statement in parent’s name AND
             a.      GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.
     
    OR 
     
    3-      An affidavit of residency (the same proof of residency listed above will be required of the owner/leaseholder for an affidavit)

     

    Proof of Legal Guardianship (if applicable) (MUST HAVE TO ENROLL):

                This is required if student is being enrolled by someone other than the birth parent listed on birth certificate.

    Copy of the child’s proof of age (MUST HAVE TO ENROLL):

                Birth certificate or other proof of age      

    Social Security Number (USED FOR FEDERAL REPORTING ONLY):

                Copy of student’s social security card (waiver option)

    Transfer Students must provide the following information: (MUST HAVE TO ENROLL):

                Proof of grade placement

                Final report card from previous school

                Name and address of previous school

    Immunization (state required): (MUST HAVE TO ENROLL):

                Georgia Certificate of Immunization/ Form 3231 must be current

    Eye, Ear Dental Screening (IF NEW TO THE STATE OF GEORGIA):

                Copy of Georgia Form 3300

    REMEMBER…….the enrollment form must be complete and all other paper work attached before a schedule will be issued.
