NECESSARY PAPERWORK NEEDED IN ORDER TO TRYOUTWe use an online program named DragonFly Max to collect all paperwork. You will only need to upload the physical and clearance forms. All other forms will be filled out online.DragonFly Max Flyer and InstructionsPlease print these and take to the doctor (these forms will be uploaded to DragonFly Max in their respective tabs). All other forms are filled out directly on the DragonFlyMax website.IF YOU NEED SCHOOL INSURANCE TO COVER ATHLETICS PLEASE CLICKEligibility:The student must have passed 5 out of 6 classes for the previous semester (end of 1st semester grades for the 2023-24 school year).The student must also be age eligible.
