• MEDICATIONS

    Should your child need to take a regular prescribed medication or an over-the-counter medication, we need you to:

     

      Provide the medication and anything needed to dispense it ( dosage cup, measuring spoon, aero chamber, nebulizer, etc.)

    ALL MEDICATIONS ( PRESCRIBED AND OVER-THE-COUNTER) MAY NOT BE BROUGHT IN BY THE STUDENT.  PARENTS WILL NEED TO BRING MEDICNE IN PERSON.

    All medication must be in original package with students name, grade, and teacher written on it.

    We cannot accept any loose pills.

    Students are authorized to carry and self-administer prescription medications provided the WRITTEN AUTHORIZATION FOR SELF-ADMINISTRATION OF MEDICATION form has been completed and signed per the doctor.

    Does your child have ALLERGIES, ASTHMA, DIABETES, SEIZURES, CP, MS, CA or any other chronic health condition??

     

    We MUST have an action plan form on file from your child’s doctor.  The forms are available in the clinic.  This form MUST be filled out by the doctor and returned to the school nurse to keep on file.  We want to assist you and your child!  These forms allow us to care for your child and treat them as directed by the doctor.  ALL medications must remain in the clinic at all times!  We are happy to work with your child’s doctor and will provide them with any information you wish.
