Attached you will find Henry County's insurance and consent form.  You will need to complete this prior to practice.

Attached is the heat policy acknowledgement signature required.  This form needs to be submitted before you participate in practice.

Attached you will find the GHSA physical form.  All athletes will need to have a physical on file to participate in school activites.  The physicals are good for one calendar year. 

GHSA requires every athlete has a concussion form on file.  Please read, review and sign this form and return it.
