This form is the Georgia High School Athletic Association Physical Evaluation Form and must be completed in its entirety for a student-athlete to participate in athletics in Henry County.Henry County Extracurricular Consent FormThis form is required by the Henry County Board of Education. Student and parent signatures are required.This form details the dangers of concussions and explains the required procedures that will be followed in the event of a concussion. Student and parent signature are required.This form explains the GHSA policy regarding the procedures followed in regards to extreme heat. Student and parent signature are required.This form explains the regulations that must be followed regarding the preseason practice for Football.
