Please vaccinate your student early and beat the "Back to School" rush!!!

7th Graders. This is a very important Immunization Requirement. All students entering the 7th grade are required to meet the following:

(1) Have one dose of Tetanus, Diptheria and Pertussis (Tdap); and one dose of Meningococcal Conjugat Vaccine.

Please contact your health provider to receive any vaccinations and to obtain the Georgia Certificate of Immunization - Form 3231.

An updated 7th Grade Immunization form will be required before the student is allowed to attend classroom instruction. Please let us know if you have any questions.
