Oh Boy! Field Trip!!!

What Fun! Your child is going on a Field Trip with the Class!
With all the excitement sometimes Headaches and other ailments can pop-up unexpectedly! We want to keep your child healthy and happy while away from school.
If you would like to provide Field Trip medication, we must receive a SHMA form authorizing us to do so. Also, we must have a separate form for each separate medication. A parent or guardian must bring medication to school the day of the field trip and given to the Teacher or designated School Official who will be with the students while they are away from school. Please remember, your student is not allowed to bring medication to school. All mecicine must be brought in by a parent or guardian.
If the Clinic is already storing rescue medication for your child (Inhalers, Benadryl, EpiPens, etc.) they will be given to the Teacher or designated School Official who will be responsible for them while on the Field Trip and no further action is required by the Parent or Guardian.
PLEASE REMEMBER, ALL MEDICATIONS MUST BE PICKED UP BY A PARENT OR APPOINTED ADULT AFTER THE STUDENTS RETURN TO SCHOOL AND BEFORE SCHO0L IS DISMISSED FOR THE DAY. NO FIELD TRIP MEDICATIONS CAN BE KEPT AT SCHOOL AFTER THE TRIP IS OVER.
