All students Entering 6th Grade Must Meet the Following Immunization Requirements:

Your 5th Grade Student will need an Updated Georgia Immunization Certificate (form 3231) to enroll in 6th Grade
- It must show that they have had an active case of chicken pox, labratory proof of immunity or that they have received 2 doses of the Varicella Vaccine (chicken pox vaccine)
 - It must show that they have had 2 doses of the measels vaccine or laboratory proof of immunity
 - It must show that they have had 2 doses of the mumps vaccine or laboratory proof of immunity
 - It must show that they have had 1 dose of rubella vaccine or labratory proof of immunity
 - It must be marked with an expiration date or marked as 'Complete for School Attendance'
 - It must be signed and dated by the doctor or Health Departent and must show their address and phone number
