Below, you will find the forms needed for participation in the Athletic Program at Dutchtown Middle School. For the 24-25 school year, Henry County Schools will start using Aktivate for all physical uploads.

Please note: All students trying out for any sport will be required to have a current physical exam. Physicals are valid ONE year from the exam date, students with expired physicals cannot participate in practices, games, and/or tryouts. There are no exceptions to this rule. Students must also have a current insurance policy to participate. This information should be uploaded with all physical documents on Aktivate. If you do not have insurance, feel free to contact our athletic director to receive information on sports insurance policies.
