Items Required BEFORE Trying Out:

·                   Completed Physical Exam Form uploaded completely to Dragonflymax.com or download the app from the App Store/Google Play and upload Physical 

·                   Completed Insurance Waiver, Medical Eligibility Form, Health History Form, and Cardiac Arrest, Concussion, and Heat Policy Forms (all completed on Dragonflymax.com or on the Dragonflymax app)

·                   Must have passed 5 out of 6 (this includes Connections classes)
