Instructions for athletic paperwork (Aktivate)

Go to Aktivate and create account to get paperwork.

All athletic forms are now digital and can be filled out on line, the only exception is the physical from your Doctor must be scanned in.

 You can not do conditioning or tryout without having all forms completed and either submitted online or printed and brought in

Athletes of Luella Middle school will be focused on development of fundamentals, sportsmanship, and enjoyment of recreational competition. However, the ultimate goal of all coaches and administrators is to positively develop the character of Student First Athletes.
