     PHYSICALS & INSURANCE
     
    To participate in athletics at OMS, every student must have a current sports physical on file. We will be using an online program named Aktivate to collect all paperwork.

    *You will need either 24 hour coverage or at school coverage.  Please print a copy of the confirmation and policy # and upload to HC Consent Form on Aktivate with your physical.*
