• Please see the links to the left for mandatory forms that you must have in order to participate in athletics (including voluntary conditioning) at Woodland High School.  All forms must be turned in together at the same time to your coach. 

    Please make sure all forms are signed and dated and all information is completed on each form. Incomplete forms will be returned and students may not participate until all forms are complete.
