Safe and Drug-Free Schools and Communities Survey
Over the next few weeks, ELMS students will be participating in our annual Georgia Health Survey, which is entitled the “Safe and Drug-Free Schools and Communities Survey.” This is a completely anonymous Schools and Communities survey that asks for responses that pertain to student involvement in substance abuse, violent situations, school climate, and nutrition. All Henry County Middle Schools administer this survey to their students, and our school will begin administering this in February.We encourage all of our students to participate in this survey, as this information is important to us as a school; however, parents do have an opportunity to have their student to “opt out” of participating in this survey. If you do NOT wish for your child to participate in this survey, then you may complete the Passive Parental Permission Form (CLICK HERE) and return it to the school office by February 1st. Parents may also view the Health Survey as well as pick up a copy of the Passive Parental Permission Form in our front office. The completed “opt out” form must be returned to the office by February 1st if you do not wish for your student to participate. Any student who does not turn in this form to “opt out” of the survey will be asked to participate in this survey. If you do not wish for your student to participate in this survey and you wish for them to “opt out” of taking it, then please click on the following link and complete the Passive Parental Permission Form.
